AdultProphy Care includes: Once yearly exam and x-rays (provided on initial exam date and yearly after that date), 2 prophy cleanings yearly done on a 6-month frequency, and 1 Emergency visit. 20% off all dental treatment. (***20% off does not apply to include Invisalign and Bleaching).

Gum Care includes: Once yearly exam and x-rays (provided on initial exam date and yearly after that date), up to 4 periodontal maintenance cleanings per year completed no sooner than a 3-month frequency, 1 fluoride treatment and 1 Emergency visit. 20% off all dental treatment. (***20% off does not apply to include Invisalign and Bleaching).

Advanced Gum Treatment Plus includes: twice a year exams and x-rays (provided on initial exam date and yearly after that date), 4 quadrants of SRP (scaling and root planing) completed as the first treatment type after purchase, 3 periodontal maintenance cleanings per year completed after SRP and no sooner than a 3-month frequency, and 1 Emergency visit. 20% off all dental treatment. (***20% off does not apply to include Invisalign and Bleaching).

On last periodontal maintenance visit we will determine which program you are able to move to for renewal (new contract is to be signed when moving to another maintenance program).

Program Guidelines

• The Tammy Tran DDS Inc Dental Savings Plan gives our patients 12% off dental services for 12 consecutive months starting the day the plan is purchased.
• Payment is due in full on the day of service.
• This savings plan cannot be used in conjuction with another dental plan.
• This savings plan cannot be used in conjuction with any office promotions.
• Non-refundable
• Excludes dental products (toothbrushes, toothpaste, whitening gel, etc...)
